Job Description:
A company is looking for a Patient Care Advocate II to
coordinate patient services and support programs.
Key Responsibilities
Coordinate in-home services for patient referrals and complete
patient enrollment
Handle inbound and outbound communications while maintaining
service levels
Document issues and resolutions, and assist with work distribution
and planning
Required Qualifications
High school diploma or equivalent
1-3 years of relevant work experience
General PC knowledge including Microsoft Office and Internet
Ability to adapt in a dynamic work environment
Willingness to work a flexible schedule for peak call times
